Dynamic fields in a form

Hi,

I need a form with fields from a table and some dynamic fields taken from other tables. Is it possible to have fields other than from a database table?

I need to show all the values from a table as label in a form and i need to get content for each labels. But i need to store each label as individual record in the DB. Any solution?
